I scoured Pinterest for a Father's Day questionnaire and wasn't satisfied, so I made my own and decided to share! Click here to get it. I left the year blank so you can use it more than once. Also, there are two per page, so they are small enough to fold in half and inlcude in a regular greeting card or nice to paste on colored paper if making a homemade card.
